but it came back after I restarted it and windows rescanned it (can't remember the exact wordage). Question is, is my HD dying?

Assume it is. Make a backup immediately and keep an eye on it.
I've been in this business a long time, I've never heard anyone say "Man, am I sorry I made a backup". That's like a lawyer saying "Man, am I glad my client talked to the police".

try downloading speedtest from speedtest.com? install it and run it and click on the "exotics" tab and it will show how much life is left in your hard drive if the # is low <25% then i would suggest back-up of all data and buy a new one especaially if its making noises, may eb on its last leg. if in the warranty period usually 1-3years return it back

You better backup your important files while you can. It sounds like your HD is going south fast.

The Click of DEATH! Seriously back up your data and get a new HDD.
